Certification Requirements and Apps


You may access and/or download the eligibility requirements, exam information and application form for the following -

  • Primary certification in Family Practice 
  • Recertification in Family Practice
  • Certification of added qualifications in Geriatric Medicine
  • Recertification of added qualifications in Geriatric Medicine
  • Certification of added qualifications in Sports Medicine
  • Recertification of added qualifications in Sports Medicine
  • Recertification of added qualifications in Addiction Medicine
  • Recertification of added qualifications in Adolescent Medicine
  • Certification of added qualifications in Hospice and Palliative Medicine
  • Certification of added qualifications in Sleep Medicine
  • Certification of added qualifications in Undersea and Hyperbaric Medicine

The residency pathway requires that your AOA-approved training must be approved as complete by the American College of Osteopathic Family Physicians (ACOFP) before your final scores are released by the AOBFP and before presentation to the AOA with a recommendation for certification.  That approval requires submission to the ACOFP of residency paperwork.  The ACOFP may be contacted at (800) 323-0794, (847) 952-5100 or at www.acofp.org (under 'residents' selection) for details.

If ACGME-approved training has been completed, AOA approval must be obtained.  Contact the AOA Department of Trainee Services at 800-621-1773.

The American Osteopathic Conjoint Sports Medicine Education and Evaluation Committee may be accessed at www.aosmcee.org for details on the approval of sports medicine fellowship training.

All completed applications and supporting documents, with the exception of those in sports medicine and undersea/hyperbaric medicine, should be sent no later than the postmark deadline date to -

Four weeks should be allowed for verification and processing of materials.  It is highly suggested that materials be sent via certified mail with return receipt requested or priority mail with delivery confirmation to allow for confirmation of delivery with post office.
